• Copy of child's immunization report/vaccination record
• Copy of birth certificate
• Copy of doctor's report in case of any medical conditions

Important Note:

Parents should inform the nursery management of any previous surgeries, joint dislocations, potential food allergies or dairy product allergies (phenylketonuria), asthma or chest allergies requiring vapor inhalers, febrile seizures or above-normal body temperature, hereditary diseases such as epilepsy or similar conditions for consideration.
