(If the report is not in English, please provide a translation with the original version.) • Immunization records. • A copy of the Qatar Residence Permit (QID) for both the applicant and the sponsor. If this is not available at the time of application, please complete the form to indicate "in process"** • Passport-sized photo (optional). • Any relevant medical reports or educational psychology reports if available.</p><p>Step Two: You need to create an account on the school's website if you don't already have one.</p><p>Step Three: From the account homepage, select the "Start New Application" tab and complete all sections of the form, including uploading all mandatory documents and making an electronic payment of QAR 200.</p><p>All relevant information must be disclosed, including details of any disciplinary, social, physical, medical, or psychological issues.
